PARENT-STUDENT COMMITMENTS TO EVELYN MACK ACADEMY
These are the admission requirements for Evelyn Mack Academy, I understand the following:

1. The school has the authority to discipline my child as deemed necessary (including spanking). If this type of punishment is not acceptable, please advise the Administrator of punishment that you feel is appropriate.

2. The administration has complete responsibility for disciplinary action and placing my child academically.

3. My child has permission to take part in all school activities including sports, field trips, etc., for the entire school year. I will not hold the school liable for injury (injuries) incurred while at school, during a school activity, including sports and field trips, etc.

4. The school must operate on sound financial policies. We expect all payments to be made on time as designated by the tuition contract. The policy concerning payments is outlined in The Parent/ Student Handbook.

5. If a disagreement or problem arises that involves our child, we will not complain or criticize to another parent, but will seek a solution through the chain of command, teacher first, then Mrs. Evelyn Mack.

6. Students and parents are responsible for the damages to school property.

7. The school reserves the right to dismiss any student whom:

a. Does not respect school regulations.
b. Does not cooperate in the education process.
c. Failure to comply with the standard of attitude and conduct establish by the administration.
d. Alcohol control substance abuse or cigarette smoking on school property is prohibited. School Administrator has the right to search my child if there is probable cause suspected of any of these abuses. Parents or law enforcement officer will be contacted in case of illegal substance.

8. As supporting parents of Evelyn Mack Academy, I am/We are expected to uphold the School and authority in both word and deed before my child. Failure to comply with this principle will create division and cause rebellion in the student.

9. Should the time come when I can no longer support the policies of Evelyn Mack Academy, I will politely and quietly withdraw my child, upon approval and review by the Administrator.

10. As a student, I will refrain from drugs, alcohol, smoking and profanity.
